None of this is mandatory — a referral without it still gets seen. It simply lets us book the right appointment length and order anything missing before the patient arrives.
- Imaging already done — and where, so we can retrieve it rather than repeat it. See the imaging-before-referral guide →
- Conservative care tried — physiotherapy, injections, analgesia, weight and activity modification.
- Urgency — a plain sentence is enough; it changes where the patient sits in the list.
- Insured or self-funded — so the rooms can quote accurately at first contact.
